Seattle Children's is seeking a Registered Nurse (RN) Case Management for a nursing job in Medina, Washington.

The RN Care Manager works to improve the health of and reduce the cost of care for children. The Care Manager is responsible for assuring program-specific transitional care, ED visit follow-up, and PCP co-management strategies are implemented for eligible families at the most appropriate site of care, including hospital, home, school, and PCP office. Interventions include enrollment and risk assessment of the target population, development and implementation of shared care plans and coordination of care, and staff/physician education related to the care coordination process. patient advocacy, promotion of a collaborative practice model, appropriate use of resources, and patient/family education. Partners with PCP practices to promote collaborative management, proactive communication, and oversight of adherence to expected patient treatment plans and outcomes, and engagement of patients and families. Supports the data collection and outcomes tracking of individual patients and the program overall.
  
*Required Education and Experience*
BSN degree or equivalent combination of education and experience.
Minimum of five (5) years of recent clinical experience.
Experience in community-clinic settings, community outreach and collaboration with care teams.
Experience in case management serving complex children and families.
Per the driver's license requirement noted below, must have a driving record acceptable to Risk Management.

*Required Credentials*
Current Washington State Nursing License.
Current Washington State Drivers License.
Current BLS for Healthcare Providers.

*Preferred*
Masters Degree in nursing, public health, or healthcare administration.
Pediatric clinical experience and expertise.
Experience in continuity of care/discharge planning, community based care, population based care management.
Bilingual Spanish.
Experience and skills that demonstrate proficient customer service and organizational abilities.
Health education experience.*Compensation Range*
$55.42 - $83.13 per hour*Salary Information*
This compensation range was calculated based on full-time employment (2080 hours worked per calendar year). Offers are determined by multiple factors including equity, skills, experience, and expertise, and may vary within the range provided.
