RN Case Manager

StartDate: 6/23/2025 Pay Rate: $2000.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS – WA RN license; AHA BLS; 1+ year RN Case Management experience, with experience in a long-term acute care setting; 6 hours of Suicide Prevention Training completed through the WA DOH website.

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – Monday - Friday 8a-430p. Weekends on request. (40hrs/wk)

START DATE – ASAP
